Equipment Reclamation Guide
This guide outlines the process of facilitating an IT equipment return for a caregiver that no longer has access to PH TECH systems.
This can occur for both immediate and retroactive terminations.
It is the responsibility of both the core leader and IT to facilitate the equipment return.
Notify the caregiver that upon their employment end date, all IT equipment must be returned to PH TECH. This can be done via:

How to locate a caregiver's personal email address
Search Zendesk for "New Hire - First Name Last Name"
Personal email addresses are communicated directly to core leaders during the new user onboarding process via email.

If you have already returned all PH TECH owned equipment, great! You can disregard this email.
If you still need to return equipment, please use one of the following options below:
Schedule a time to return PH TECH owned equipment to an office location.
If you qualify for equipment return shipping service, please use the shipping form to create a request.
